General Information
Description of ServiceThe Awendaw Nutrition Site offers yoga classes, exercise equipment, and four evidence-based wellness programs:
--Living Well/Chronic Disease Self-Management Program (CDSMP) - an effective self-management education program for people with chronic health problems [arthritis, diabetes, lung and heart disease, etc.] that meets two hours per week for six weeks;
--Arthritis Foundation Exercise Program (AFEP) - a recreational exercise program that covers a variety of range-of-motion and endurance-building activities, relaxation techniques, and health education topics which also benefit persons without arthritis;
--Arthritis Foundation Self-Help Program (AFSHP) - an effective self-management education program, consisting of two-hour sessions offered weekly for six weeks, that helps people with arthritis learn and practice techniques designed to reduce pain and decrease the number of visits to the doctor; and
--Matter of Balance - a wellness program, consisting of eight two-hour sessions, designed to reduce the fear of falling, stop the falling cycle, and increase the physical activity levels in older adults.

TerritoryAgency serves eastern Charleston County, including the areas around Awendaw, Isle of Palms, McClellanville, Mt. Pleasant, and Sullivans Island.
Public TransportationAgency provides transportation to and from the three meal/activity sites for participants. Public transportation for a fee is offered by Tri-County Link Bus.
Intake ProcedureTo become involved in senior center activities, call or visit the closest center. To apply for home-delivered meals and/or home living support, contact the agency's main office in McClellanville.

EligibilitySeniors, age 60 or older, are eligible for services. Spouses and dependents with disabilities who are younger than 60 may qualify for some services.
